Natural does not mean interaction-free. Some supplements change how medications work, some add to what medications already do, and stacking several products can compound in ways you did not intend. This article covers the three mechanisms behind almost every real interaction, the specific ones worth knowing by name, a category almost nobody warns you about, and the honest limits of what anyone — including us — can tell you.

This is general education, not personal medical advice, and it is not a complete list. A pharmacist or physician can check your specific medications and health history — and is the right person to ask before you start, stop, or combine anything. Never stop a prescribed medication because of something you read here.

The three ways interactions actually happen

Interactions sound mysterious until you see that nearly all of them fall into three plain mechanisms. Once you know which one is in play, the practical response is usually obvious.

First, absorption. Some supplements physically bind a drug in the gut so less of it gets into you. Minerals are the usual culprits: magnesium, iron, and zinc can all reduce the absorption of tetracycline and quinolone antibiotics, and magnesium and iron can interfere with levothyroxine and bisphosphonates. This is the friendliest category, because the fix is almost always timing — separate the doses by a few hours and the problem largely goes away. It is also the category people most often get wrong, because taking everything together in the morning feels tidy.

Second, metabolism. Your liver clears most drugs through a set of enzymes, and a supplement that speeds up or slows down those enzymes changes the drug's effective dose without either of you touching the bottle. Melatonin is a clean example: fluvoxamine and other CYP1A2 inhibitors can sharply raise melatonin blood levels. Resveratrol carries a possible interaction with drugs cleared by the same cytochrome P450 system. There is no timing trick for this one — it needs a clinician.

Third, additive effect — two things pushing the same lever. This is where the serious ones live. Several calming botanicals add to sedatives and alcohol. Several supplements add to the bleeding risk of anticoagulants. Serotonergic supplements add to serotonergic drugs. Nothing here is exotic; it is arithmetic. The supplement is doing exactly what it says on the label, and so is the drug, and the sum is more than you planned for.

The interactions worth knowing by name

These are drawn from cautions documented across our own ingredient library. They are the ones where the stakes are highest or the mechanism is most specific — not a complete inventory.

  • Vitamin K2 and warfarin — the most direct interaction in our library. Vitamin K is precisely what warfarin and other vitamin K antagonists work against, so supplemental K2 can meaningfully reduce the drug's effect even at low doses. This is not a "discuss it sometime" caution: it requires physician involvement and INR monitoring. The awkward part is that K2 is usually sold alongside vitamin D3 for bone health, to exactly the older demographic most likely to be anticoagulated.
  • Ginkgo biloba and blood thinners — may add to bleeding risk with warfarin, aspirin, and clopidogrel, with rare bleeding events reported. Its monograph caution also covers stopping one to two weeks before surgery.
  • 5-HTP or L-tryptophan and antidepressants — both are serotonin precursors, and combining them with SSRIs, SNRIs, MAOIs, triptans, tramadol, or dextromethorphan carries a serotonin-syndrome risk. For 5-HTP this is an explicit Health Canada contraindication, not a soft suggestion. The two supplements are also redundant with each other.
  • L-tyrosine and MAOI antidepressants — contraindicated, on hypertensive-crisis grounds. L-tyrosine may also interact with thyroid medication and levodopa.
  • Huperzine A and cholinesterase inhibitors — huperzine A works by the same mechanism as drugs like donepezil, so the combination compounds cholinergic load. It also carries cautions around beta-blockers and other heart-rate-lowering medication, and can slow heart rate itself.
  • Ashwagandha and thyroid, sedative, immunosuppressant, blood-pressure, or diabetes medication — plus the liver signal below, which is the more serious issue.
  • Sedating botanicals and CNS depressants — valerian, passionflower, chamomile, and others add to benzodiazepines, alcohol, opioids, sedating antihistamines, and anesthesia. Stacking two or three calming supplements on top of a sedative is the most common way people quietly overshoot.
  • Minerals and antibiotics or levothyroxine — the timing problem described above. Separate the doses; do not simply abandon either.
Ashwagandha deserves singling out, because the concern is not a drug interaction at all. NIH's LiverTox classifies it as a likely cause of clinically apparent liver injury, a 2023 case series documented ashwagandha-related liver injury including deaths in people with underlying liver disease, and Australia's TGA issued a liver-safety alert in February 2024. Anyone with a pre-existing liver condition should avoid it. This is why we hold ashwagandha at Grade C regardless of how good its stress data looks.

The category nobody warns you about: your lab results

Interactions are not only supplement-versus-drug. A supplement can also interfere with a test your clinician uses to make decisions, which means the harm arrives as a wrong conclusion rather than a side effect.

  • Creatine raises measured serum creatinine — the marker used to estimate kidney function. The evidence does not indicate kidney harm in healthy people, but if your clinician does not know you supplement creatine, a raised creatinine reading can be misread as declining kidney function, and that misreading can trigger real medical decisions.
  • High-dose folate can mask a vitamin B12 deficiency — correcting the blood picture while the neurological damage of the underlying deficiency continues undetected. This is a documented reason to be careful with high-folate products taken alone.

Both are avoidable with one habit: tell your clinician what you take. Not "some vitamins" — the actual list.

Higher-attention situations

If any of these describe you, the default should be to check before adding anything, rather than to check only if something feels wrong.

  • Taking blood thinners or blood-pressure medication
  • Taking antidepressants or other psychiatric medication
  • Taking thyroid medication, or antibiotics alongside any mineral supplement
  • Being pregnant or breastfeeding — the majority of ingredients in our library carry a caution here, usually because the safety data simply does not exist
  • Having a scheduled surgery — several supplements affect bleeding or add to anesthesia, and typically need to stop one to two weeks ahead
  • Having a liver or kidney condition
  • Managing a chronic condition, or taking multiple medications

Why "no known interactions" is not the same as "safe"

This is the most important paragraph on the page, and it is the one most sites leave out. When a product — or a database, including ours — says no interactions are documented for a combination, that almost never means the combination has been tested and cleared. It usually means nobody has studied it.

Supplement-versus-supplement combinations are especially under-researched, because there is little commercial incentive to fund a trial whose best outcome is discovering a problem. The absence of a documented interaction is an absence of evidence, not evidence of absence — and the gap is widest precisely where people are most adventurous, stacking four or five products nobody has ever studied together.

We hold ourselves to this rule explicitly. Our ingredient pages and Stack Audit report documented relationships only — pairs where our reviewed copy already says something. We never synthesise an interaction claim from mechanism or inference, and we never present a quiet result as a clean bill of health. If we have not published a note on your combination, that is a statement about our library, not about your safety.

What to actually do

  • Keep one list of everything you take — prescriptions, over-the-counter drugs, and every supplement, with doses. Photograph the labels if that is easier.
  • Show it to a pharmacist. This is genuinely the highest-value, lowest-friction move available to you: pharmacists are interaction specialists, the consultation is usually free, and no appointment is needed.
  • Tell your surgeon and anesthetist too, well before the date — not on the morning.
  • Separate mineral supplements from antibiotics and thyroid medication by a few hours rather than abandoning either.
  • Do not stack multiple sedating or multiple serotonergic products without guidance. Redundant mechanisms are where the arithmetic turns against you.
  • Add one thing at a time. If something goes wrong with four new products started at once, you will have no idea which one did it.

The bottom line

Most supplement interactions are not mysterious — they are absorption, metabolism, or addition, and most are manageable with timing, spacing, or a short conversation. The genuinely serious ones are few enough to name: vitamin K2 against warfarin, serotonin precursors against antidepressants, sedating botanicals against sedatives, anything bleeding-adjacent against anticoagulants, and ashwagandha against a compromised liver. The rest of the risk lives in the unstudied middle, where confident reassurance from anyone — including a database — should be treated as the marketing it usually is. Keep the list, show the pharmacist, add one thing at a time.
